E.g., if your name was 'Jane Anna Bloggs', your username might have this format - 'ja3-bloggs'.

The password for your username will have been emailed to you when you first started at UWE & you will have been asked to change it.

Once given, you can withdraw your consent at any time using the Removal of Consent form.

I consent to the processing of my personal information as described in the UWE disability and wellbeing services privacy notice for the purpose of giving me counselling or mental health support. I understand that my personal information will:

• Only be shared without my permission when required by law, if there is a serious risk of harm to me or others, or I have breached UWE regulations or policies.
• Be kept for up to 6 years after the end of the academic year in which I last used your services.
• Only be shared with third party organisations in anonymised data format.
